Sign Your Waiver

1. Covered activities

I request permission to participate at Lost Soul MMA, operated by Cage & Caliber LLC, in activities that may include martial arts instruction, striking, kickboxing/Muay Thai, grappling, wrestling, takedowns, sparring, bag and pad work, conditioning, resistance training, supervised drills and use of training equipment, to the extent offered and permitted by staff. Activities may involve physical contact with instructors or other participants.

2. Specific risks

I recognize and understand inherent and other risks including falls; collisions; punches, kicks and joint manipulation; head, neck and spinal injury; cuts; bruises; sprains; broken bones; concussion; eye and dental injury; illness or infection; aggravation of a medical condition; and, in extreme cases, permanent disability or death. Safety rules, equipment, instruction and supervision reduce but cannot eliminate risk. Other participants may act unpredictably. I agree to stop and tell an instructor if I feel unsafe, injured, dizzy or unwell and to follow reasonable instructions and posted facility rules.

3. Fitness and medical decisions

I am responsible for deciding whether I am sufficiently fit to participate, seeking independent medical advice when appropriate, and providing staff with emergency information and relevant safety restrictions. I understand that instructors are not providing medical diagnosis or treatment. I will not train while impaired by alcohol or other substances. I may decline a drill or sparring round and notify staff of concerns.

4. Voluntary assumption of inherent risks

I freely choose to participate and knowingly assume the ordinary and inherent risks of the activities described above, including the risk of accidental injury caused by normal participation. This acknowledgment does not excuse a party's conduct when law prohibits release of liability.

5. Release of claims

To the maximum extent permitted by Michigan law, I release Cage & Caliber LLC d/b/a Lost Soul MMA and its owners, employees, trainers and agents from claims for bodily injury or property damage caused by ordinary negligence in connection with my voluntary participation in the expressly described activities, including negligence relating to facility conditions, supervision or equipment, but not gross negligence or willful misconduct or any claim that cannot legally be released. I understand this is an important legal waiver of rights and have had the opportunity to ask questions before signing.

6. Emergency care

If I appear unable to consent during an emergency, I authorize staff to contact emergency services and provide available emergency-contact and pertinent information. I understand staff are not medical professionals and do not promise any particular emergency intervention. This paragraph does not create a general advance medical directive.

7. Photos and advertising

Training participation does not automatically permit use of my photo, voice or likeness in public advertising. A separate optional consent is required for promotional use. Ordinary security camera footage, if any, is handled under the Privacy Policy and posted notices.

8. Agreement and signature

I am at least 18, have read this document, understand its legal consequences, and sign voluntarily. Michigan law applies to the extent permitted. If a portion is unenforceable, the rest may remain in force where allowed.
